    My shop address has changed. What do I do?

    Please provide Field Service with a memo on company letterhead listing the old address and the new address. If your phone and fax numbers have changed, please list the new numbers as well. We will also need a W-9 form and Blanket Certificate of Resale with the new address. This information can be faxed to 321-726-4417, attn: Lartesha Jorden.
